WELLBEING MEASUREMENTS
A4 sheet to monitor how you are. Aranya produced this one below on his Permaculture Design Courses and it led him to some realisations about himself. I just don't get on with this sort of diary keeping. There's an edge to explore there for me especially as I need to recommend others do this. See below for the blank.
PAtient Symptom trackers.
I have got links below to a system on symptom monitoring I ask my herbal medicine patients to fill in. I love it as it shows a pattern of health over time.

I got the idea of being able to evaluate a project in terms of MONEY WELL SPENT from a conference I went to called Growing Health. I am interested in horticultural therapy and health via gardening so was impressed to see projects that could say to their Clinical Commissioning Groups (them that decide where the money goes that they were twice as effective than counseling and cost three times less with data. So below is a great link and do explore the website. Ideally you design this before you start and you can use qualitative interviews and testimonials too but the idea of saying it was £x per person and had this benefit is just great. I hope to create a model intervention design plans in future and this links well to DESIGN 6 - Community Health Clubs. and DESIGN 6.1 Peer Mentoring

So Sit comfortably and answer the follow questions. You will get a score and this can be compared at different times, Ideally before and after. Of course it isn't perfect but perhaps asking a person a simple open question with it like "how well do you think this questionnaire reflects how you are feeling right now?" may explain and make sense of how useful it was.
USE WELLBEING SCALE FOR QUANTATIVE DATA.
What effect does a Community Health Club/ Peer Mentoring Scheme/ Gardening on Prescription have on my Wellbeing?
USE SHORT INTERVIEW TO GAIN OTHER OUTCOMES (QUALITATIVE)
What benefits do participants say came from the intervention?
What benefits do Healthcare professionals see form the intervention?
WORK OUR COST PER PERSON OF THE INTERVENTION TO EVALUATE COMPARED TO CURRENT COSTS.
